Hi,
I'm facing the issue that one user gets an approval multiple times despite the approval action only runs once.
I've already checked that the flow only ran once and the user is only once in the recipient field.
The action itself shows no sign that it ran into a re-try.
Did anyone else experience such a behavior?

I ran into the same issue today,
the advanced settings of this block (dots on top right ... -> settings) offered me the option to set of the Asynchronous Pattern, which lead into a single approval notification.

Hello, I'm also having the same problem of multiple intermittent Approval Request sent out even though the response has been done. Is there a fix for this or any work around?

In our MS Service Health from yesterday evening:

Final status: We've determined that an unexpected increase in requests to the infrastructure supporting email functionality for approvals occurred, causing the impact. We’ve optimized the impact infrastructure and increased its request processing limit, which we confirmed through telemetry to have remediated the impact. Scope of impact: Your organization was affected by this, and any user may have been impacted. Start time: Monday, March 27, 2023, 9:30 AM (8:30 AM UTC) End time: Monday, March 27, 2023, 4:50 PM (3:50 PM UTC) Root Cause: An unexpected increase in requests to the infrastructure supporting email functionality for approvals occurred, which caused the infrastructure to become unbalanced, process requests inefficiently, and resulted in impact. Next steps: - We're reviewing this unexpected increase in requests to understand why it occurred to help develop better automated strategies to detect and proactively remediate impact from such request spikes, preventing similar impact from reoccurring. This is the final update for the event.

My approval steps in some flows are also showing the following error - in edge and firefox
Http request failed: the server did not respond within the timeout limit. Please see logic app limits at https://aka.ms/logic-apps-limits-and-config#http-limits.
In firefox all setps are showing the following error - it is not appearing in edge

From the Issue description, I understand that your facing issue with approval flow and it retries number of times sending multiple approvals out for one approval. It times out and flow is getting failed.
