Action Timed Out - Approvals

Hi, while I was on maternity leave, a flow I had written for my firm started to fail. It is designed to check a sharepoint list for names added there (from a different flow) during the current week, create a unique list, then send approval to each person on the list. It's failing at the part where it sends approval with the below error and I can't figure out why. It actually does not fail until the 30 day timeout period, which is even more confusing to me. Please help - thank you!

I have just noticed that there are some people who make it through the first part only to fail at this part, where it is supposed to add the approval response to an excel document:
