Leave approval flow originating with Forms

Hi,

I am trying to create a flow which originates with a Staff leave form ( which may include an uploaded doc such as a medical certificate). I have created the associated List in our SharePoint Intranet site but I have 6 different managers that they need to choose from to request approval.

I have tried several different flows but they all fall apart at the approval stage because I cannot enter the "Response" equals Approve. I have tried several other similar response options that are available but none seem to work.

Basically I need my flow to start with Form - Responses added to SharePoint list - Manager choice results in a switch. Each request to each manager then starts an approval flow with a Approve/ Denied/ Amend option. Approve reply is sent to the requester email and payroll, Denied and Amend is returned to the requester email.

I can easily build a Form response to create a SharePoint list and then create a second "approval flow " but its the approval section that keeps failing after the switch condition ... I haven't even gotten to the part where the approvers can view the attachment 😞

Hi,
I have tried several different type of flows but this one is the closest to being successful.

The CASE works and selects the correct manager – I receive an email but it doesn’t appear to register any of the responses that I try to enter. Each time I get a notification as per attachment image.

The flow continues to “run” in the history and I get no acknowledgement email of my response of Approve/Deny/Amend.
