Hi,

I am hoping to set up a delay to send an email to the approver if approval is not approved by a certain time.  For testing purposes I've set this for 2 minutes

Do I need to set the 'Delay' should run after to one one of the other options, except 'Is Successful as the email is still sent when the approval is made within the time limit set??

To make this work, you need to use the Create an approval action rather than the Start and wait for an approval action. Then follow the Start an approval action with a Wait for an approval action. You then want to put the Delay parallel with the Wait for an approval action. The way you have it now, no reminder will go out until you get a response from your approval.

However, even with this, your reminder will always be sent even if the person responds before the delay. So you should put the delay and e-mail into a Do until action and use a variable in the Do until that checks if a response has been received. Here is an example from one of my flows:

Note that the Do until action by default times out after 60 minutes, which you will want to change. In my flow, the approval needs to be done within 24 days. I send a daily reminder, so my delay is 24 hours, and I modified the timeout limits in the do until action to a count of 24 or a timeout duration of P24D, which is 24 days.

To the right under the Wait for an approval action, you will see two Set variable actions. The is the Boolean variable used to determine whether to send the reminder in the do until action. It changes the default value of true to false to stop sending reminders.
