I have an approval in a flow with the most mind-boggling issue.

When certain users approve, the flow does not show that anyone has approved and is infinitely stuck waiting for an approval (see attached screenshot). The oddest thing is that there is an action that populates a SharePoint list field as approved AFTER this approval action and it actually runs even though the flow is stuck on the approval. And no actions after that one run.

This flow was originally made sometime last year and there was never any issues with this approval until around several months ago when this suddenly started happening

I've tried deleting and recreating these approvals and the issue persists. Has anyone experienced anything like this?

This appears to be a backend approval orchestration issue rather than a flow logic problem. The approval succeeds, but the callback fails to rehydrate the original flow run. Moving away from “Start and wait for an approval” and enforcing unique correlation data will resolv the issue.

Stop using “Start and wait for an approval”
Replace it with the modern pattern:

Create an approval

Wait for an approval (v2) or

Wait for a response to an approval

This decouples creation from waiting and avoids the stuck‑state bug entirely.

This is the single most successful workaround reported by the community.
