Having trouble to add attachments from different questions MS form into planner task

Hello, 
I have an MS form which has 4 sections each section with the option to upload a file. I need a way where I can pull all the uploaded files from the different questions and add them to a task in the planner. I got it working but only with a single section. Does anyone know what I need to do to get the flow to work?
