Why am I receiving multiple approval requests from Teams Approval action item (trigger: when new response is submitted ms forms)

Hello all!

 

This is odd and has never happened before, but I have a trigger of when a new response is submitted on MS Forms (but I don't suspect this to be involved in the issue). I have the Teams action item 'Start an Approval' later on that ends up sending 2 (or sometimes even more) approvals to the person under the 'Assigned To:' field in that action item. Does anyone know why this is happening and how I can fix it to only send 1 approval request to the person that the requests are assigned to?

 

The flow works perfectly besides this. The only reason it failed was because I was running a test with adding 2 users from the MS Forms response to a security group, and 1 user didn't exist in the tenant, but the other user was added (this was only for testing purposes).

    HI @jharville ,

     

    If you click test the flow, it sometimes gets triggered twice. (maybe it's a bug when testing)

    Not sure if this is the cause of your problem.

    The weird thing is that even when I just leave the flow turned on, and submit a response on MS Forms (not through clicking 'test' on the edit page), it'll still submit 2+. I've been getting 2 requests as of late, but got 6 at one point earlier today. Do you know a way around this?

    I have no idea...

    Could you check the flow run history list?

    Does it have some duplicate running history?

    Besides, are there duplicate flows in this or other environments?
