I'm ashamed to say how much time I've devoted to this problem. But it's too much. I've used Chat and Co Pilot and they have both led me into the depths of automate hell. 
 
Creating, deleting and re creating and redeleting. Still my workflow doesnt do what it's supposed to. I've officially broken up with AI. 
 
This is my last ditch effort here. 
 
What I need to do:
 
1. We are now using 365 online forms for our employee compliance quizzes. 
2. The forms (Quizzes are done)
3. I created workflow with conditions, so that if an employee fails, (gets below a 90% score), the employee gets an email. And, HR gets an email. 
4. I f they get 90 or above, they get nothing. 
5. In the sharepoint file, my HR team needs to see total score converted to total % becasue apparently forms to excel doesnt do this on its own. So, i have created new columns for "Total %" and a "Pass/Fail" column on that shared file. with formulas. 
6. This worked long ago in the beginning stages of my workflow hell. But, because we need a way to acccess the employee form in the excel shared file because you cant search forms by quiz taker name because the form is to to people outiside of the company. 
 
I'm not sure what kind of file i can include here for assistance? Just wanted to give the parameters first. HELP PLEASE

    Forgot to add my "ask". Emails to hr and the employee are not going through if an employee fails. This used to work and doesnt now. I saved archived file when it was working, and tried to import and it wouldnt accept. So i've had to start from scratch multiple times. 
     
    In our industry, we HAVE to be able to go to a completed quiz and pull the entire quiz contents, not just the date from a spreadhseet saying they pass or fail.

    Please share a screen shot of the flow and how the actions are configured. Also tell us what error message is generated when the workflow fails. Based on your "ask" it is important that your screen shot shows how the e-mail actions are configured that send the e-mail to HR and the employee. Without those things, one can only guess what may be wrong.
     
    In your original post you asked about something needing to be a percentage. Did you resolve that issue? If not, show in the screen shot where and how you are setting this. Also show the configuration of the SharePoint field that is storing the value. Is it a text field, a number field, etc.

What do you mean by this? Do you realize you can go into the individual results in Microsoft forms to see how each person responded and save it as a pdf?
     
    Since you said you "HAVE" to have this, I see this as your only option as it sounds like a regulatory requirement. To get an approximation of this in a workflow, you would need to create a Microsoft Word template, populate the responses into the Word template, and then save the file. But since that would not be the original response, it would probably not hold up under an audit. Therefore, you would probably want to print responses directly from your form.
     
    And don't be sad about being in HR. I started my career in HR and now I am in IT.

    Review answers is the same thing but for quizzes and allows you to print the quiz with the answers and the score. Click the three dots under the score to open the print menu.
