How to build a dynamic list form within SharePoint 365

Hey, I'm hoping someone can help!

Background: I need to create a data entry form for colleagues to complete via our SharePoint site. The form will feed into a list that needs to be exported daily. The form needs to be dynamic, in the sense that if Question 1 is answered with Option A, the subsequent questions are tied to Option A.

There are a few parts of the form that need to be dynamic, with the subsequent questions displaying based on whatever they answer in an earlier part of the form.

My company have locked out Microsoft Forms functionality, hence attempting this using lists. I really hope I've explained that correctly but here is a more illustrative example..

Example:

Question 1: What is your name?
Question 2: Date of form entry
Question 3: What type of form entry is this?
        - Option A
        - Option B
        - Option C
Question 4 (variable):
         - You selected Option A, XYZ
         - You selected Option B, ABC
         - You selected Option C, ETC

And so on.. I've searched high and low online to try and find an answer to this but can't find anything specific. I do have Power Automate available to use, but I can't find the commands relating to lists specifically so really need some help if this is an option that works! Thank you in advance

You can use Microsoft List forms for this (which is like Microsoft Forms but integrated within your SharePoint library/list).

And that feature also supports conditional branching. So, no need to use Power Automate for this.

Below is an example adapted to your scenario, if that helps?

    1. List Forms option

    2. Example conditional branching setup

    Happy to help out 😁
