I am looking for a template that I can revise that staff can use to create a Safety Report for a workplace.
It needs to have a Form that can be completed by anyone and then sends a notication for someone to review, comment and approve.

You could create a form in the Microsoft Forms, and then you could create a flow as below:
When anyone submit a response for the form, the flow would be triggered and sends an email for someone to review, comment and approve.

Any tips on how to make the Approval email be nominated in the form? Or make it the line manager? I am looking to create a form, responses get approved, but it needs to be by their line manager. Is this possible?
