Condition Met, but action is greyed out

I have a flow that is working properly up until the condition is true.  When the condition is true, then Create Item is grey out.

When the Condition is false, the action is skipped (acting correctly)

    @12strikes In your first screenshot, the reason your Apply to Each action is greyed out is because there is nothing to loop through. You'll need to share the full screenshot of your flow to provide context.

    Hi..
    Little more details about the process..
    I'm pulling data from a PowerBI data source.  if the ID is already in my SharePoint list , then skip the record. IF the ID is not in the SPL, then add the record from the PowerBI Datasource.  In the Filter Query TITLE is from SharePoint, and ALLCOMBINDED is from PowerBI
    The Condition is 

    length(outputs('Get_items')?['body/value']) is equal to 0 
    (True means the record in PowerBI does not exist in SPL)

    Here is a run that was "Successful" but the record was not created in SP

    @12strikes It's not ideal to have the Get Items action nested inside an Apply to Each action. You should pull that action OUTSIDE of the Apply to Each action and run it only once. Then use a Filter Array action to filter out the item(s) that match your criteria.

    In this video tutorial I’ll show you 3 practical ways to use the Filter Array action and how to use it properly.

    Did you know that the Condition action has a limit of 10 conditions? Although it might look like the Filter Array action can only accept one condition—this is not true. By using the advanced mode you can enter multiple conditions into a Filter Array action with an expression.

    As mentioned before, the Apply to each action isn't "running" because there isn't anything to loop through. You can see the Get Items action hasn't returned any items—this is why the Apply to Each action isn't running (nothing to loop through).

    @12strikes The reason this flow run ran the Apply to Each action successfully is because the Get Items action has returned items.

    If you take a look the flow run that didn't run the Apply to Each action, it's because no items were returned. Which is why you see [] in the outputs. This means that the Get Items action didn't return any items.
