I'm trying to use approval workflow when i tested it the test is successful without sending any approval email

It seems that this item does not contain an attachment, so the actions in apply to each will not be executed. It is an expected result.
Please ensure that attachments have been added to the item when it was created.
Or you could add a Delay below When an item is created, set the delay time to 15 seconds and check if the action in apply to each will be executed.
