I'm new to using PA. I using Forms to collect the data an store responses in Excel and then using PA to send an email if all 3 conditions are met (also collecting responses in another Excel). The issue I'm having is when I select all 3 conditions they are blue and checked, but when I select save they immediately unselect themselves. I have 4 conditions. The flow shows a green banner saying "your flow is ready to do. Try testing." When I test, it follows the NO actions when I did test using all 3 conditions. Can anyone provide assistance?

The checkboxes are used to make a group.
Check two of them and click ellipsis and choose Make group.
Leave them alone. You have to meet all three requirements will the condition returns true.
So check outputs of your "Get form details" for these questions' answers to verify which one failed the condition.
