MS Form pre-filled URL broken

Dear Team - We are in the process of triggering survey notifications using MS Form to the users from Power automate. We get the report from ServiceNow tool and our flow will read those details and send the survey to the users. 
 
There are fields like Ticket no, Short description, Analyst name, user name and email address where these values are Pre-filed in the MS form by reading the same from an excel sheet using power automate and the Pre-filled URL gets generated which will be shared to the users as email notifications.  
 
The problem is, sometime there is a break in this flow where the short description value does not gets properly filled or only half filled and due to this the Pre-filled URL also gets broken. When we click on this URL, we see form has only partially filled values and incomplete. 
 
We tried to manually get the Pre-filled URL with same short description value directly from the Form it gets generated successfully and we could all values are pre-filled properly. Could you please let me know why the has been broken from power automate and is there any cause of this failure.
 
Note: We use the short description value as below
Others - I am doing subway upload The one output "l1625_3_02.rtf" I got failed during import. I have attached a screen shot for the same issue. Could you please look into the issue with option 7 faliu
