Duplicate SharePoint List items from Form Response

Very new to this. I have a flow that takes Form responses and turns them into items in a sharepoint list. Right now when a Form is submitted the flow generates two items instead of one. However, one of the "duplicates" has a blank "Title" field and a blank choice column, while the second entry appears to be exactly how it is intended to be. Here is my flow, any ideas?

Hi @cmurfree
Do you want to save MS Forms multiple choice to SharePoint List?
It is assumed that the name of the choice field in the list is 'Answer' and the choice question in the Forms is 'Please select your answer', and they both support multiple selections.
You can refer to the following screenshots to configure Flow.
When a new form is submitted like this:
The new item will be created in list.
