Send Email with Options: Inconsistent behaviour

Hi all, im finding some inconsistent behaviour with "Send Email with options" and wondered whether anyone could shed any light on it. Please note, I would love to use the approvals flow, however, i cannot due to external users and the restrictions on that organisation and you cannot alter the connection of your approvals actions so im stuck with this, any information would be appreciated :). 

Here are the options i am using:
 


This is in an attempt to have the Actionable Message card be the only option send to users. I wanted this because I've discovered when the HTML message is used (see below) then the response from "Send email with options" is incomplete.

When this email is received by a user, a choice made will only ever send this output:

When a user gets the Actionable message card like this:
 
The response is either correct like the documentation says, or it can also be missing information like the HTML message



All im doing for these tests are sending the email with options to myself. Im not changing anything in the email with options, or my inbox and its still giving wildly inconsistent results. Is this normal or am i missing something fundamental to how this all works? In the docs it says the responses should be like this:

Is anyone has had more experience using this and found a way to make it more consistent, or allow you to determine when a user actually clicks on these things and its not just their out of office triggering it, please let me know.

    Did some further investigation and testing. Following this guidance: 
    Office 365 Outlook - Connectors | Microsoft Learn
    "If Use Only HTML Message is set to No, and Hide HTML Message is set to Yes, only the action card buttons will show."

    I changed my action to the following: 


    I manually sent 15 to myself. Out of those 15, I had 8 that had the actionable card (those that are marked succeeded: 
     
    Checking the body of those 8 with actionable cards only 5 returned the SelectionOption, UserEmailAddress, UserTenantId, & UserId.

    Im changing nothing about the flow, just sending it back to back. Why is it varying SO much?
