Canvas App attachment not appearing in Model-Driven App Documents subgrid

Hi everyone,
I just started learning about power apps and flows.

I have a Canvas App where users submit a record using an Edit Form and attach a PDF file through an attachment control. I already have a File column in my Dataverse table.
The table is in Dataverse, and I have already enabled SharePoint integration. I also have a Model-Driven App built on the same table.
My goal is:
Upload the attachment to SharePoint
Have it linked to the Dataverse record
Display it in the Documents subgrid in the Model-Driven App

The file should be uploaded to SharePoint (using a flow), but it does not appear in the Document library and in the  Documents subgrid in the Model-Driven App.
My questions:
What is the correct way to associate the SharePoint document with the Dataverse record so it shows in the Documents tab?
What configuration step is commonly missed in this scenario?

Any guidance would really help. Thank you!

    Can you share me the screenshot of your flow.?

    can you please validate below points
     
    • In DV you configured sharepoint?
    • From Flow did you created SharePoint Document Location Table?
    • Did you mapped the same Path from Step 2?

    There are two things to check here:

    1.Documents subgrid not appearing
    Even if no files exist, the Documents tab should appear once SharePoint integration is configured.
    Please re-verify under:

    Advanced Settings → Document Management → Document Management Settings
    • Ensure your Dataverse table is selected 
    • Ensure the correct SharePoint site is configured 
    • Complete the wizard if not already done
    Once this setup is completed, the Documents tab/subgrid should appear on the Model-Driven App form even if no files exist yet.

    2. File not appearing in Document Library
    If the tab is visible but files aren’t showing, then check your Flow.
    Make sure the file is uploaded to the correct folder path
     
    If uploaded to the incorrect folder, it won’t appear in the Documents library.
