Ticketing System in SQL Server with Power Automate & MS Forms

I want to create a ticketing/idea system with the front end using MS Forms and the back end in SQL Server. I would also like to auto-populate the form when certain criteria are met (they type in their name, and it auto-fills their department and supervisor) from a different datasource on the server. 

 

Is this possible?

    MS Forms, pre-populate? No. But, you CAN get all of those things via the Flow after it's submitted.

     

    If you want to pre-populate, you're going to want to use PowerApps, and you are in premium territory.  But, you will be in premium territory regardless, with SQL.
