Troubleshooting: Power Automate Microsoft Forms to Teams Planner Task - generating two cards instead of one

I'm setting up a flow to use Power Automate to trigger a new Teams Planner Task when a Microsoft Form is submitted. I can't figure out why the flow is resulting in two Planner cards being created (one with a response field from the Form - Title of the Request and the other is the ID that is automatically generated for the Form. Can anyone help? Thank you!!

    Hi @awestcott ,

     

    Do you have another copy of the flow that is running?  Please check your cloud flow runs because there isn't anything I can see in your flow that would duplicate the 2nd card (other than another flow).  Have you adjusted anything in the trigger settings?
