I have an instant cloud flow where end-users fill out a MS Form, which then creates a sequential approval. The flow & teams notifications have been working for months up until 1st week of November...

1st Approver Receiving Power Automate email notification and Teams Activity Notification, but 2nd and 3rd Approver in Sequential Approval not receiving Teams Activity Notification anymore (2nd and 3rd approver still receive Power Automate Email Notification)

Have tried the following:

Checked the notifications option is enabled on Create an Approval Action
Checked that end-user / 2nd and 3rd approver have notifications enabled in Teams
Rebuilt the flow from scratch

There's a manual work around whereby going into the MS Teams Approval App and clicking "Follow Up" will send a Teams Activity Notification to 2nd or 3rd approver, however this is not ideal.
