Create Issue in Jira on Microsoft Form response - error in connector?

I can reproduce the same issue and I fixed with this process (important I don't know about the JIRA I only worked with this for that case to try to find the solution)

First I removed the report field from this form

After removed that the Action work fine

Other solution is you can make the parent field optional and that resolved the issue.
