Error with Power Automate Workflow Approvals

I've followed countless documentation as far as trying to find a solution to this error I keep receiving. Power Automate to create a more efficient way of managing invoices via Microsoft Teams Approvals. When a user hits yes to the "approval" button we would like for it to reflect the result of that approval into F&O. Allowing users to approve invoices directly from their phones. I will attach a screenshot of my build, as well as the error message I'm receiving. While using some documentation from Microsoft Learn, I've came across some helpful flows to help me with my current build, but I keep running into this same error in regard to my last step "Execute Action." 

Action 'Execute_action_1' failed: An error has occurred. Exception occurred while executing action complete on Entity WorkflowWorkItem: Invalid outcome was provided that is not one of the available outcomes for this work item. Exception has been thrown by the target of an invocation.

Since you don't show the configuration of the action in the Apply to each, can you explain what you're doing in it and what is it connected to? Also, what is the action within the apply to each? I do not recognize the icon.
