BadGateway Error only on "New" Form - Edit OK

Hi Folks -

 

Just recently I started to get a "BadGateway" error when users try to submit a New Request.  However, when submitting from the Edit Form, it works without issue.  Does anyway have any idea?

I tried to delete the DataSource and resource it but no luck.
