Show save button outside of overflow menu (ellipsis)

I deployed an update through pipelines that failed. Ran deployment again and then noticed that the save and save and close button on all forms are now inside the overflow menu. Additionally, the user and status used to show up on the right side of the header, now they appear on the left. Rendering in dev is correct, the problem is in production. 
 
I have spent upward of 40 hours troubleshooting this. I've used the form designer to republish all form. I tried force show from the app designer command menu. Updated and republished views. I've republished, turned modern controls off and on in both environments. Turned modern controls off and on at the application level. Hours and hours reading, using copilot and google AI,  Tried to use XRMToolbox (unsuccessfully), using the  &flags=FCB.ModernCommandBar%3Dfalse  to the end of a form url to force the classic command bar, but nothing seems to work. 
 
I am at a loss and completely frustrated! Please help!

    Quick steps you can try now:
    • In Prod, open the app on a wide monitor, set zoom to 100%, and hide the nav pane (if possible).
    • In App Designer → Edit command bar (Main form), temporarily hide your noncritical custom commands and publish. Check if Save appears on the main bar.
    • If yes, add back custom commands one by one to find the width tipping point.
    • If you must always show Save, add a custom “Save” command (Power Fx or JS) and place it early in the bar; give it a short label (e.g., “Save”).
    • Review the form header in the modern designer. Ensure the intended fields are selected for the header and the header flyout is enabled as needed.

    Model‑driven apps now use the modern command bar. OOB (out‑of‑box) commands are not freely editable, and the bar automatically overflows items under space/priority rules; when the framework decides there isn’t enough room, commands (including Save / Save & Close) are pushed into the ellipsis.
