I am O365 E3 user, I created a powerful automation, but a diamond icon appears next to the name. Does this mean I need to pay extra for the process or ignore it?

That means it is a premium connector and requires a Power Automate per user license to use: https://learn.microsoft.com/en-us/power-platform/admin/power-automate-licensing/types#standalone-plans

It's still working fine now but I'm worried about hidden fees because the flow is running every day. It would be a shame if there were hidden costs.

Premium means that your licence is not enough. I think you need E5 version.
