Model App - Quiz

I am trying to create a dynamic quiz (assessment) in model driven app. Admin should be able to create different question types (short answer, long answer, single choice, multiple choice, matching). 
 
Can you please guide on how I can do this? Also how it would be then visible to the learner? Example: Single select to appear as radio buttons and multiple choice as check boxes. 
I dont want to use too many grids, rather give a seamless interface.- something like Microsoft forms.
Also, each question to have its own attachment. 

Any guidance would be helpful.

    You can use Customer voice surgery it will be like MS form and its store all information in dataverse. Also, if you want to create user friendly UI in model driven then you can use custom pages as well. it will be like canvas UI in model driven app and modify accordingly.

    Model-driven apps are very good for handling data and admin-focused tasks, such as creating quizzes, managing questions, manging quiz response and maintaining relationships in Dataverse. However, they are not well suited for delivering a rich, dynamic learner experience out of the box.
     
    Approach:
    Model Driven: (Data Management - Used by administrators to create and manage quizzes, define question types, configure option,handle response and everything)
    Canvas App or Custom Page in Model Driven (Respondent Experience): Used by respondents to take the assessment, where questions can be rendered dynamically using appropriate controls such as radio buttons, checkboxes, and text inputs. (As Model-driven apps offer limited flexibility for dynamically rendering different question types and layouts at runtime, which is commonly required in assessment experiences.)
