Date picker control is distorted in list forms

Hi 

 

Date picker control is disorted in list forms. Any idea what needs to be fixed. Check month year and date. tried with different options not working

    I've seen this when enabling some of the experimental features. In my case, I had to turn off Enable Modern Controls
