Check if item exists in List - if yes, update item, if not create item

I know this seems very simple but I can't get it work correctly. Users complete a form that populates a SharePoint list. They would complete this form several times and I want their item to update if the same email address is entered. If not, I want a new item to be created. I have it working to update the item, but when I add a new item, it creates multiple entries. Any help would be great, thanks.
