Copilot Studio agent (ICM Buddy Premium Test) consistently returns “Sorry, something went wrong. Error code: SystemError” on every interaction, including simple inputs like “Hi”, when using the Test your agent panel. The same thing happened to another agent already published on teams, in this case the error message happened both on the test panel and on teams...

    Did the agent ever work, or has this occurred since the very first conversation prompt you tried? If not, I would suggest you create a brand-new agent by clicking on + Create blank agent. When it opens, test it right away by saying hi.
    If you get the system error here, it may be an environment setting, DLP policy or some other issue with your environment. I would contact one of your admins so they can check the environment settings. If the agent responds, then it is probably something you did while building it, but without seeing what you've done and how you've configured the agent, it will be challenging to diagnose through the community.

    We also have recently seen a similar issue in our environment where random users will start receiving error when engaging with one of our agents that has been working smoothly with relatively no issues since October of last year.   They now randomly will get "Sorry, something unexpected happened. We are looking into it.  Error Code SystemError Conversation ID X..."  

    We have our On Error topic configured to escalate but even the On Error topic is not getting triggered when this SystemError occurs.   I am seeing it for the last 2 weeks.

    This “SystemError” in Copilot Studio (Power Virtual Agents) is a platform-level issue, not something you caused. Here’s what you should do:
    1. Check Service Health: Go to the Microsoft 365 Service Health Dashboard. Look for any incidents affecting Copilot Studio, Power Virtual Agents, or Microsoft Teams.
    2. Restart Agent: Try unpublishing and republishing the agent. Sometimes this clears transient errors.
    3. Check Environment: Make sure your environment (Professional Premium Dev) is active and not in a suspended or limited state.
    4. Review Agent Settings: If you recently changed authentication, connections, or data sources, revert those changes and test again.
    5. Test a New Agent: Create a brand new, simple agent in the same environment. If it fails, the issue is with the environment or platform, not your agent.
