How do I get CDS contacts to appear in Outlook?

It is an Add in that you can add from withon Outlook itself or from the O365 admin center. Currently, It shows OLD enviroments only, enviroments created after spring update will not be shown. The add in itself is nice but it works only with the standard entities, not with the custom entities, which is a limitation.

The Dynamics 365 App for Outlook can be used to synchronize contacts between Dynamics 365 and Exchange. It is currently not enabled for CDS organizations, but we have it on our roadmap.
