Flow sending wrong confirmation email

Hi - I've tried creating a cloud flow in a few ways (forward an email and send an email with file content) and unfortunately it keeps sending the wrong confirmation email (from another users inbox, not the one I've set up). I can't isolate if it's an issue with the Form and Form ID or if I have something incorrect in the flow.  Any ideas?

    Can you share a screenshot of your current flow setup? This will help troubleshooting your issue.

    We ended up creating a new form and a new flow.  My colleague deleted the flow that was running on her end.  We couldn't isolate the specific issue, but this resolved the situation for us.  Unfortunately, we had 47+registrants on the original form, but will manually send the confirmation and link the updated form with correct flow in future reminders.
