I'm trying to create a simple Flow to create cards in a Trello board whenever a response is made to my Form.
I'm using the same Microsoft Id for both Forms and Flow, but when I try to connect to Forms from Flow, I get this error message:
Please check your account info and/or permissions and try again
As mentioned above, I'm using the same account for both products, so that's not the problem. What permissions are needed and where do I configure those? Exposing an actionable error message would be really helpful!
I've tried deleteing and recreating both the Form and the Flow multiple times; I can successfully connect to Forms using my work account, but this stuff isn't for work and I want to use my personal account. I'm an Office 365 Home subscriber, if that makes any difference.

It would have saved me many hours (at least six) of attempted troubleshooting if this was reflected in the error message itself.

So I assume that the form using your personal account info could not be detected by flow.
Please consider creating a form under your work account.
By the way, when sharing images with us, please remove/hide your personal info for security purpose.

Checking under Data | Connections, everything looks like it has connected properly. My work account connects just fine, but it's my personal account that has the Form I want to use. Here's a screenshot:
I've already read the page you linked, along with extensive searches and reading of the community forums.
Since the account information is correct - and connected - what permissions are required, and where would I configure those? These details don't seem to be publicly documented.

Before creating the flow, have you created a connection to Microsoft Forms successfully?
Please check the connection and make sure the connection status is Connected. Please go to Data>Connections to check the status of a connection.
