I have an On-Prem SQL Database and need to query it from Power Automate.

Yes, you will need an On-premises Gateway, which will require a premium license. This article will walk you through all the steps required. SQL Server On-Premise and Power Automate (jondoesflow.com)
That is correct, a data gateway will be needed. It’s a simple install on a on-prem server (dont install it directly on the sql server). You’ll probably also want to create an App Registration as your connecting account and ensure all endpoints from the Power Platform Infrastructure IP list are whitelisted through your firewalls.
