Create a Form from a list of questions (ID, Section Name, Question) in Excel

I'd love to very quickly create a simple form based on data in an excel sheet. Currently the excel rows are different questions (there will probably be 1000 +/-200). Each row contains an ID#, a pretty ID (id-sectionAbbr), Section Name, Question. I want each of these to be a question on a form with the Name including the pretty ID.

How do I do this?! Pulling my hair out.

 

Conversely, I'll be happy to set this up in PowerApps if that's easier.

    @Anonymous with that number of questions there is no way you can use Microsoft Forms but you will need a data source. I would add the questions into a SharePoint list (copy&paste using Quick Edit) and then have a Power Apps form and filter the list of questions down either via cascading dropdowns or from the starting letters of what you type into an input field. You could also use the spreadsheet as the data source in Power Apps without using SharePoint, either by importing the spreadsheet or having it as a data source on OneDrive.

    But I hope you're not expecting your users to answer 1000 questions - some of them have homes to go to!

    I wouldn't worry too much about them. I'm trying to automate a testing process for new software. Looks like I'm writing software for the testing the software!
