Form to Excel Start

Hello there, 

 

I am having issues with the form to excel flow I am working on. Everything is set up and working besides columns A-E. ID, Start time, Completion time, Email, Name. This information was showing when the form was initially set up and now the columns are not being filled.

 

What can I do to fix this??

    @Anonymous  Is your Form anonymous submission? Or I suppose was it switched into anonymous? I would have thought [ID] would still be collected though.
