Duplicate entry being created

Hi everyone. 
 
I am using Power Automate to submit a Forms document to a SharePoint list. 
When the form is completed and submitted, it is creating a 2nd entry that is blank. 
 
Would anyone know why this might happen?  Thank you for the support!

    This seems easy enough to solve but need to see some of your existing flow please.  Can you paste in a couple of images of your flow (not upload as attachments)? Thanks in advance!
