Creating a survey and embed it in an email

Hello,

I know I can use Power Automate to create a flow that sends a recurring email every month.

However, the issue is that this email have to consist of a survey with 2 questions. 1st question is a choice option with options 1,2,3 and 4. Whereas 2nd question is a open-ended comment section. (like the picture). Is this possible?

Hello @a_elbaroudy ,

My issue is to embed the survey/form into email for users to do and submit via email. Is that possible?

Also, is it possible to do a survey via flow?
