Help Creating an Automated Process for Alerts in Teams

I need help creating an automated process to share alerts in a Team of suspects who have stolen. There should be some type of form for someone to fill out that includes an image of the individual that is then posted in a Teams channel. The location selected in the form should send the alert to a specific channel.
 
Example: Onalaska would go to the Western Market channel, West Allis would go to the South East Market channel. 
 
Please provide thorough detail on how to accomplish this and how I should get ahold of you if I have questions.
