Power Automate flow running but not moving/working

I have created a flow for Leave Requests at work. For some reason, some of the flows don't always go through. 

For example, an employee completed the form which triggered a confirmation email to be sent to him, and for a request to be sent to the manager. He received the confirmation email, however the manager has said that she did not receive the request. 
When looking at the flow, I can see that the status is waiting, but I am not sure why the request has not gone through to her? It has worked before and I know she has received them. 
How can I get this flow to continue working without getting the employee to complete a new form?

    @nels99 It's hard to offer any recommendations without seeing your full flow and the logic behind it. If you are using the New Designer, toggle it off and click each action to expand it. Upload a screenshot of your flow in edit mode.

    Hi @nels99 

     

    To me, without seeing your flow as @creativeopinion  mentioned. Its very possible you are being throtteld. When that happens, flows will dead stop at whatever action was happening when it throttled.

     

    Is this a test environment or a dev environment?

    What is your license type?

     

    This exactly describes what you would see. I don't know if this one is just triggering too much, or if you have a bug where its stuck on a Variable, thats being used in a Loop, but the loop isnt a closed loop (concurrence on, max set to 1)

     

    There are not a ton of reasons why it would stop. And if others stop, or the flow seems to stop responding, that would lead even more to a capacity issue.
