I am trying to build revision Form , My scenario is . I have Newform is for adding new record in SharePoint List . Also I need Revision form for my requirement. revision form need to have same field as Newform. But title field is drop down .

That drop down can choose title from same list titles . when i choose any one title, rest field should pick automatically. and if i made any changes and save them it shouldn't be create new record. it should update same record what is choose one .

Please help me . is it possible to do ? if Possible please give me idea and some codes
