Hello everyone, maybe someone can help me in solving one issue

Еhere is a form created for example the Institute (in which all the data is filled in). When creating a project, you need to add these Institutes (there may be several of them, unfortunately Lookup is not suitable)

    Show me your data source and the app configuration, a sample data is better.
