Create Approval and SP list with attachment when Form is submitted

I have a form for people to fill out with attachment. The Form resides in the Team Environment and saves the attachment directly to the Shaprepoint Documents/App/Microsoft Fairfax/'Form Name'/'Question'. Once the form is filled I want it to create item in Sharepoint List with the attachment and create Approval with attachment in the same Flow. The difficulty I'm having is with the attachment. I can't seem to grab the attachment at the time when the form is submitted.

You could try this way to get the attachments from the response of the form.

I gave your idea a try but not sure how to use what you suggested for items being stored directly into the SP shared document and not One Drive.
Instead, I tried this. It gave me an error saying route did not match.

I solved it. I just changed the get file attachment to get file attchment using path and it seems to work so far.
