We are using the generic unmodified flow Microsoft SharePoint provides when you click the ellipse to the right of the file > Automate > Request sign-off.

We have 4 to 6 users using this flow 10 to 20 times a day.
The flow works up to the point where the Approver approves the request it shows up in Teams, but intermittently does not return the email from the Approver to the Requester.
Four to Six users send these requests to multiple users, multiple times a day. For those same users, sometimes the email gets returned to the Requester and other times not.
