Above is the screen cap of the issue. It occurs on all lists, and within the app a red x exist on data tables that are filtered subsets of these lists.Is Microsoft going through some back end hiccups??
Experiencing the same issue this morning. Refreshed my app and suddenly everything connected to MS SQL started throwing this error. Nothing changed on the server or app connection, just refreshed.

I just started getting this too!
I am unable to refresh data sources, add new data sources.
I even tried creating new canvas app from scratch and adding a datasource and it fails exact same error as you are seeing.
Did this ever get fixed for you?

I am having thew same issue. Was developing an app connected to SQL server and suddenly giving this error:
Error from token exchange: Bad authorization header scheme
Cannot refresh the data source or create new apps using this data source connection...
I used another browser and it worked. Then a day later I tried my usual browser and it worked again too.
Just came across this while working on my app. Here is how I fixed it.
- Logged out of my Microsoft account in the browser (in my case Firefox)- Closed all browser windows
- Reopened browser, Logged back in to Microsoft account
- Hard refreshed browser (Hold Shift + F5 or Hold shift and click refresh icon on the browser)
- Opened app
Error is gone now.
