Multi Level Approver Problem

Set up a basic approval flow for two approvers. The process seems correct however, the first approver always results in disapproved for the end result. Everything looks correct. Using Outcome equals approve but results are always disapproved in the flow. Any recommendations?

Can you please post a screenshot of your flow, with the configuration of your Approval (and any outputs from there) shown?
