Please try below steps to troubleshoot:
1. Save this flow as a new one.
2. Use this original flow, delete the send approvals action, add a compose data operation and use manager email as the input.
3. Use this failed history to test and run the modified flow, check the results of the compose action to see if any email will be lost.
