Form submission to SharePoint list item: two items created instead of one?

Hello all, 

 

I wonder if anyone could help me with this issue: I created a form and then an automate flow so that for each submission:

1. A SharePoint table item is created, -- where the problem lies

2. An email is sent. - run smoothly

 

The full flow looks like this:

I spent some time to get the columns of multiple choices enabled by creating two variables and mapping them as below.

Once this is done, the flow was able to run successfully but instead of one item per submission, I got two; one with the multiple choices, one without.

I cannot figure out what went wrong.. Any idea would be greatly appreciated.

    @RitaHL 

    Any chance that you have two Flows?  Maybe you have one that you were testing and didn't disable/delete?

    To test, disable your Flow and submit a new Form.  Is an entry created?
