MS Forms to SharePoint List - new list item for each question response

Hi everyone

 

I am trying to create an Audit form in MS forms which automatically adds tasks in an SharePoint list when an issue is found.

 

A question is presented and the user has 3 options:

 

1) Compliant (with regulation/question)

2) Finding (noncompliance)

3) Observation (compliant but suggested improvement)

 

the above is the same for every question in the form.

 

i have built a flow that can add a new list item in a SharePoint list called ‘actions’ for an individual question…

 

However, I want to replicate this process for each response in the audit form, where the user has responded ‘finding’ or ‘observation’

 

is there a simpler way to apply this flow for each response in the form which answers either of these options?

 

As a further level of complication, I would also add attachments and descriptions (other questions in the form), per response.

 

again, I can do this simply on one response, however I imagine there must be a better way to scale it across a form with 20+ audit questions?

    To automate task creation in a SharePoint list based on "Finding" or "Observation" responses from your audit form, you can use Power Automate with an "Apply to each" loop. This loop checks each form response and, if marked as "Finding" or "Observation," creates a corresponding item in your SharePoint list. You can also handle attachments and descriptions by mapping these fields to your SharePoint list during the flow.
