I have a issue that I don't know how to solve on

Hey @marys1612 . Not sure what the problem is. But I will suggest you to break the flow and find which field or input is responsible for the error.

@marys1612 Couple things
why do you have apply to each operation when the trigger is a new submission? There should only be one submission.
as @Sundeep_Malik suggested, let's isolate the issue and find which field is causing the error
that assigned by column does not look right to me, the claims: shouldn't be empty
can you show us the full inputs for the create item operation

The most likely issue IMO are some complex columns, such as person/choice/attachment in your list that is causing the issue

Thanks for the suggestions.

Turns out that the flow didn't like multiple dynamic inputs in the notes section so have had to cut this down to one.
