I have built a flow connected to a SharePoint List from O365 and it goes as follows:

1. User submits a request on the SharePoint List

2. an approval request through flow goes to his/her manager for approval/rejection

3. when Manager approves/rejects a notification email is being sent to the user cced HR for the confirmation

4. Approval Status field in SP list gets updated

in this point I ma facing an issue, as the user only gets a rejection email notification even if the manager approves and the field is always rejected !

In your condition statement change Responses to Outcome. Responses is a collection of Responses to the approval even if there is only one approver. Outcome will summarize the overall approval or rejection. With one Approver it will be either Approve or Reject

Please compare the Outcome dynamic content with Approve in the Condition and your problem could be solved as @ChristianAbata and @Pstork1 said.
